A pre-defined list of entries for user to select from. A common scenario, in which user needs to select one of the multiple entries to report the observation or communicate an occurrence during the service execution. This pre-defined selection form can guide users through the process of recording all the minor details.

- List of entries (Just like a drop-down) listed in a simple form format.
- Option to select “Other”. On selection of “Other” a text box should be displayed with option for user to enter the details of the entry.
- A multi-line text box should be displayed at the end of form to let user enter more details about the selected entry.
- The multi-line textbox should be optional and configurable as part of the component properties
- Skin and look of the component shall be exposed as configurable property for rebranding purposes.
- Label (Title) of the form shall be configurable from the properties of the component.
